NEW Registration Process for PSR/PEP

***Please read entire packet carefully.***

How to Register

This year, we will have two ways to register your child in our Parish School of Religion (PSR) or Preschool Education Program (PEP).  The traditional way will be to send these forms, along with your payment to the Church Office marked PSR/PEP Registration.  (411 Springfield Pike, 45215)

We will also be holding a Registration Weekend on August 4 & 5.  After all three weekend masses, you will have the opportunity to register your children for the PSR/PEP Program and sign up to volunteer for the various needs of the program. Classes begin on August 26.

Payment Information

The following fees are for the academic year 2007-2008.  Please note – there is an additional $10 sacramental fee per child for grades 2 and 8.  Please add this to the amounts listed below.  Make checks payable to St. James Church/Religious Education.

 1 in PSR and/or PEP                 $100.00

  2 in PSR and/or PEP                 $175.00

  3 or more in PSR and/or PEP    $225.00

Deadline for Registration

Registration materials must be postmarked by August 13, and must include all forms and payment.  Any registrations postmarked after this date should include a $15 late fee.  Any registrations postmarked on or after August 23 should include a $25 late fee.

Medical Release Form                                 

Please fill out the medical release form in its entirety and return it with your registration materials.  This will help us to be sure that we have the necessary information for your child, should an emergency arise.  One form should be used for each child, so please make additional copies if necessary.

Active Parishioner Form

Please read and sign the Active Parishioner Form.  It should be returned with your registration materials.

Survey

In order to assess the needs of our program, we ask that you complete this brief survey and return it with your registration materials.

Complete and include the following when registering for PSR/PEP: Registration Form, Medical Release Form, Medical Information Form, Active Parishioner Form, Survey, and payment (check made payable to St. James Church/Religious Education).

*In order for your child to attend PSR/PEP classes, you must be an Active, Registered Parishioner.
